Occupational Therapy

  • Activity of Daily Living Assessments: Showering, dressing and personal care, meal preparation, domestic and community activities
  • Functional Assessment of the Client’s Environment: Advice and recommendations regarding home modifications for access and safety
  • Equipment Prescription: Bathroom aids and equipment, manual and power wheelchairs, pressure care equipment, transfer equipment, adaptive aids to assist with everyday activities
  • Independent Living Skills Assessment & Retraining: Detailed hands on assessment of client’s ability to complete everyday tasks safely and independently, identification of gap in skills, step by step retraining or recommendation of services to assist

Case Management

  • Initial Needs Assessment: Comprehensive assessment of the client’s pre and post injury life roles and abilities, identification of health and rehabilitation needs, clear recommendations and action plans for implementation
  • Discharge Planning: Complete management of discharge plan from acute hospital or rehabilitation service, including clear, responsive and consistent communication with the client, family, funding body and service providers
  • Case Conferencing: Facilitation of timely, client and action focused case conferences with medical and rehab team members
  • Client Contact: Tailored service for every severe injury client and their family including guaranteed regular appointments, in person, by phone, email or Skype where indicated

Report Writing

From time to time we are requested to write a report for our clients that are outside of our usual therapy or case management documentation procedures. We are happy to discuss a report request with you to ensure we have the correct experience and skills required. We have experience in writing:

  • Funding applications such as Enable NSW and Charity Organisations
  • Clinical opinions and treatment evaluations for funding bodies
  • Care plans and safe procedures for care
  • Client specific reports for medico-legal purposes
